This is the form to request a representative from Student Activities to come to speak to an audience. Requests should be made at least
10 business days prior to the desired presentation date, but no earlier than 6 weeks
prior otherwise, it may not be able to be fulfilled.
This is NOT the form to schedule an event or book a room/space. If you are trying to do this, complete the Event Application (in your organizational portal, click on Events and then "Create an Event").

Most of our presentations include a PowerPoint or Prezi and would require a laptop with internet connection, projector, and screen. Please indicate the equipment that is available for our use. Select all that apply. If you need to
explain other equipment available, please do so in “Additional Information” below.
